Put the form on a page

Open the page where the form should go in the WordPress editor.

Add a Custom HTML block where you want the form.

Paste your code into the block.

Save and publish the page, then open the live page to check it.

Needs: Your own hosting, or a WordPress.com plan that allows plugins.

Install a plugin that adds code to every page (a header and footer code plugin).

Paste your button line in the plugin's footer box and save.

Open any live page: the button sits at the bottom right and opens your form.
