What works on your Wix plan
A button that links to your request page works on every plan. The rest depends on the plan.
The form on a page, inside an Embed HTML element. Wix runs embedded code in its own fixed-size box, so the form cannot grow to fit: you size the box. If Wix asks you to upgrade first, check your plan.

Put the form on a page
Needs: Wix's Embed HTML element. Wix boxes code in a fixed-size frame, so this uses the form's own address in a frame you size.

In the Wix Editor, click Add Elements on the left.

Click Embed Code, then Popular Embeds, then Embed HTML.

Click the new element, click Enter Code, and paste your frame code under Add your code here.

Click Apply. Drag the element's bottom edge down until the whole form shows with no scroll bar.

Publish the site, then open the live page to check it.

Needs: Premium, with the site published on a connected domain.

Open Settings from your site's dashboard, then Custom Code under Development & integrations.

Click + Add Custom Code and paste your button line.

Under Add Code to Pages, pick All pages. Under Place Code in, pick Body - end.

Click Apply, then open the live site to check the button.
