What works on your Squarespace plan
A button that links to your request page works on every plan. The rest depends on the plan.
The form on any page, in a Code block. The floating button on every page, through Code Injection.
Code blocks that run a script or a frame, and Code Injection, need Core, Plus or Advanced. On other plans, use a button that links to your request page.

Put the form on a page
Needs: Core, Plus or Advanced.

Edit the page where the form should go.

Hover over the section, click Add block or +, and pick Code.

Click the pencil on the block, make sure the mode is HTML, and paste your code.

Save the page.
Your code looks like this:

A floating estimate button on every page
Needs: Core, Plus or Advanced.

Open the Code Injection panel.

Paste your button line in the Footer field.

Click Save.
